Ticket Custom Form

I am the IT Manager of a small/medium sized company. I would like my end users to fill out ticket forms instead of submitting emails. However, I want to create my own customized ticket form with fields that are relevant to my organization. The default fields are not relevant to my company and adding more fields to the existing default fields will become too much for my end users to navigate through. I would prefer creating my own custom form with the fields that are relevant to my company.
